This document includes two guides that organizations can use when preparing and designing self-assessment surveys. These guides provide preliminary considerations and quick tips for designing the questionnaire, such as the wording and order of questions, exceptions to make, and issues to avoid.
Preliminary considerations in these presentations include avoiding shotgun approaches, avoiding long questionnaires, establishing a method of statistical analysis that directs the questions’ format and character, obtaining feedback from a small (but representative) sample of potential respondents, and conducting a field trial.
